Scheduling your appointment is easy using our online appointment request tool. Our new online scheduling allows the patient to search 24/7 without having to explain their request to anyone by phone. Available appointment times are displayed with an up-to-date view of providers’ schedules.

Appointments are scheduled during regular business hours. We ask that you cancel or reschedule appointments at least 48 hours prior to the appointment time. There may be a cancellation fee if our office is not notified more than 48 hours before the scheduled appointment. To check in for your appointment and complete any required administrative tasks, we ask that you arrive 15 minutes before the actual appointment time.
We ask that new patients arrive 30 minutes prior to the appointment time to complete administrative paperwork and bring a photo ID as well as your insurance card. Having your previous medical records sent to our office prior to your visit will aid the physician with your treatment.
Our office hours are Monday - Thursday, 8:30 a.m.to 4:30 p.m. and Friday, 8:30 a.m. to 5 p.m.
Confirmation & Cancellation
If you receive a confirmation phone call before your appointment, please note the information carefully, and respond to us if necessary.
Appointments are scheduled according to each patient's needs and the availability of the physician. The time of your appointment is reserved for you. All cancellations and/or rescheduling of appointments MUST be done at least 48 hours in advance.
Patients, who cancel the day of an appointment or do not show, will incur a $50.00 cancellation/no show fee.This fee cannot be billed to your insurance company and is payable by the patient or their guardian. We will require a payment over the phone if you have had a no show or late cancel BEFORE we can schedule your next appointment.
After Hours Support
We ask that routine questions, prescription refills, and non-emergency appointments be made during regular business hours. If you have an urgent medical emergency please go to your nearest emergency room.
We are closed on New Years Day, MLK Day, Good Friday, Memorial Day, Independence Day (July 4th), Labor Day, Thanksgiving Day and Christmas Day.
The patient is responsible for his or her co-pay at the time of the services rendered. It will be collected before you see the provider. The co-pay is set by your insurance plan.
